Closed Door Pharmacist

We are searching for a dedicated Closed Door Pharmacist to join our dynamic healthcare team. In this role, you will play a crucial part in providing pharmaceutical care and services in a non-retail setting, focusing on preparing and dispensing medications for long-term care facilities, assisted living communities, and other institutional clients. The ideal candidate will possess a background in closed-door pharmacy or a willingness to learn, a commitment to patient well-being, and excellent communication skills.

Key Responsibilities:

  • Prepare and dispense medications accurately and efficiently for long-term care facilities, assisted living communities, and other institutional clients.

  • Conduct thorough medication reviews to ensure appropriateness, effectiveness, and safety of prescribed therapies.

  • Collaborate with healthcare providers and facility staff to optimize medication therapy and enhance patient outcomes.

  • Provide medication counseling and education to facility staff and caregivers on proper medication administration and potential side effects.

  • Monitor patient outcomes and medication adherence, and adjust therapy as needed.

  • Ensure compliance with all federal, state, and local regulations related to pharmacy practice and medication dispensing.

  • Manage inventory and procurement of medications and supplies, ensuring proper storage and handling.

  • Participate in quality assurance programs and continuous improvement initiatives to enhance pharmacy services.

  • Stay current with best practices and advancements in pharmaceutical care for institutional settings.

  • Maintain accurate and up-to-date patient records and documentation.

Preferred Qualifications:

  • Bachelor's of Pharmacy or PharmD degree from an accredited pharmacy program.

  • Experience in closed-door or institutional pharmacy.

  • State pharmacy license in good standing.

  • Proficiency in preparing and dispensing medications for institutional clients.

  • Strong analytical and problem-solving skills.

  • Excellent communication and collaboration abilities.

  • Familiarity with electronic health record (EHR) systems and pharmacy management software.

  • Dedication to ongoing professional development and continuous improvement.
