Key Responsibilities
- Deliver telephonic or video-based medication counseling and education to patients and caregivers.
- Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
- Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
- Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
- Document all interventions and communications according to organizational standards.
- Support population-health and disease-management programs.
What You’ll Bring
- Education: PharmD or Bachelor of Pharmacy.
- Licensure: Active U.S. pharmacist license (multiple state licenses preferred but not required).
- Experience: Ambulatory care, MTM, or clinical preferred. Hospital and community pharmacists passionate about patient interaction are encouraged to apply.
- Skills: Strong communication, empathy, and ability to build rapport virtually.
